Before you register,
please be sure we have these medical papers on file. If you are not
sure if they are up to date, please call us at 413-259-3065.
In order for your child to
participate in camp, current immunization (signed by a Dr. or
nurse) and medical exam (within the past 2 years to the date of the camp, also
signed) need to be on file at our office prior to submitting an online
registration. These are required for all sports and Outdoor/LIT camps at the
time of registration. Registrations will not be accepted without proper medical
records.
